Employee Medical and Health Form
The employee medical and health form is a document to leave on file and post around the school to make sure others are aware of any medical needs an employee might have. These forms outline the employee's medical condition and the actions to take if the medical condition occurs onsite. Included is also an action plan for employees with allergies and/or inhalers.
