Illness Policy and Report
This form is used to help clarify to parents the importance of the school's illness policy. This form is used to define whether a doctor's visit is required and whether a doctor's note is needed. This form also clarifies, when a child can return back to school after illness symptoms occurred.
Information is taken from the CDC, and DCFS.
